This section of the patient profile allows you to manage the patient addresses. Patients can also add, edit or delete their addresses from the Patient Portal.

Addresses

Add, Edit or Delete Patient Addresses.

Add an Address

You can add a new address for your patient. Patient’s can also add their own addresses in the patient account.

You will be presented with the Add Address Form to Fill out.

The First and Last Name will be defaulted to your patient’s name, but you can update it to someone else’s name if the patient will not be receiving the package personally. Example a wife may need to pick it up at the Post office, or its a child and you would prefer the name on the package is the Mom or Dad’s.

Mandatory Fields:

Address 1 – Street Address including the number.
Address 2 – Other information such as suite#, Unit, Apartment# etc.
Country
State/Province
City
Postal/Zip Code

After choosing the Country and State, it will populate the list of cities. If you do not see your city/town in the list, you can enter any information. It does not need to exist in the list.

Press Save Address to Complete.

Multiple Addresses

Patients can have many addresses because they may want their package being delivered to their home, office or they may go on vacation and require alternative addresses for delivery. You can mark one address as the default, which will automatically be chosen when you or they checkout their orders. Click the box next to the address you want to make the default.

Editing Existing Addresses

If the address you entered is incorrect, you can edit the address. Click on the Edit icon and it will load the address form. Make changes and press save.

Delete an Address

You can Delete an existing address if it is no longer required. Example, a patient moves to a new home and no future packages will be sent to the address. Click on the X icon to delete an address. A popup will appear letting you verify that you do indeed want to delete the address incase you press on the X icon by accident.
